At your home! (unless otherwise stated)
When:
Appointments are during normal buiness hours
Mondays & Wednesdays appointments are in San Francisco
Tuesdays & Thursdays appointments are in the East Bay
Timing
Visits are scheduled for 1.5 hours.
Preparation
You will be sent a packet of items to read and fill out before the visit. Please make sure to fill out the History Intake Form and complete other parts of the paperwork as appropriate.
Obtain your insurance card, be preparted for any payments, including the deposit and be prepared with any questions you may have.
Fees
Use the list below as a framework for determining your sliding scale fee. These fees do not include any lab work or herbal and homeopathic remedies. Lab work associated with any paps, blood draws, STI testing, etc will be added to the amount below. You are required to pay a minimum $100 Lab Fee Deposit if you receive any testing. Unused funds will be returned.
Initial visit:
Annual income less than $20,000 your fee is $75
Annual income between $20,000-30,000 your fee is $150
Annual income between $30,000-45,000 your fee is $200
Annual income above $45,000 your fee is $250
Followup visits:
Annual income less than $20,000 your fee is $50
Annual income between $20,000-30,000 your fee is $75
Annual income between $30,000-45,000 your fee is $100
Annual income above $45,000 your fee is $125
Payment and Insurance:
If you have a PPO it is possible your insurance will cover the cost of the appointment depending on your plan. Details regarding this are covered in the finacial agreement.
